We welcome all new patients; please see the 'How To Find Us' page for details of our location and practice area.

All new patients are given an NHS registration form a health and medical questionnaire to fill in, and an optional alcohol questionaire.

 Well Person Check

You are also offered an appointment for a well person check with our Health Care Assistant. We use this opportunity to collect information on your past medical history, current health problems, and regular medication. It is also a good time to check all immunisations, cervical smears, and contraceptive requirements are up to date. Unfortunately it can take up to 2 months to obtain your old notes from your previous Doctor so it is important we collect this information from you directly.

You can print off the registration form, health questionaire, and alcohol questionaire if you have a colour printer, and bring or post it too the practice.
