Our District Nurses or Community Nursing team operates from our premises but, like our Health Visitors team, they are employed by Bournemouth & Poole NHS Trust. They provide a wide range of nursing care and support to patients at home who are physically unable to attend the surgery and / or have a serious illness or disability. Their main areas of care include; wound care, hospital discharge assessments, palliative and bereavement care, cancer care and support and continence care and advice. They operate an open referral system. Appropriateness of referrals are assessed via the telephone prior to a home visit. The team can be contacted Monday to Friday 08.30 am to 5 pm on 01202 738812. Please note there is no answer machine on this number. |
