New Patient Registration

We welcome patients who live within our Practice area.

If you would like to join our Practice, please click on the registration link which will take you to an electronic Registration From. This will need to be completed for each member of your family.

 

Patients who are unable to access the internet should contact the Practice in the first instance for advice on registering. We recommend you register on-line as this makes the process much quicker, but you can obtain a paper copy at our Reception Desk.

 

Until you are fully registered with the Practice, you will remain registered with your previous GP.

The practice adheres to equalities legislation and we will not discriminate against you on the grounds of race, gender, social class or age.

When registering you will be given the name of the GP you are being registered with. However this does not affect your freedom of choice of who to book your appointments with.

 

Temporary Residents

Patients visiting from another area in the UK for a period of less than 3 months, and who become unwell, are entitled to be seen as a temporary resident. You will be able to make an appointment in the same way as one of our normally registered patients.

 

Change of Address or Telephone Number

Please let us know your new address and telephone number as soon as possible so we can keep your records up to date.  If you move outside the Practice area, we may not be able to continue to care for you; if you are in any doubt, please check with reception.
