New Patient Registration

We welcome patients who live within our Practice area.

If you would like to join our Practice, please ask the receptionist for a registration form(s), which will need to be completed for each member of your family. This is an opportunity for us to obtain important background medical history and arrange any repeat prescriptions, if applicable.

Please note that in order to register you will need to bring photo I.D. such as a passport or driving licence and separate proof of address, such as a bank statement or utility bill, along with your completed registration forms. Each person registering above 16 will need to be present with their I.D.

The practice adheres to equalities legislation and we will not discriminate against you on the grounds of race, gender, social class or age.

When registering you will be given the name of the GP you are being registered with. However this does not affect your freedom of choice of who to book your appointments with.

Change of Address or Telephone Number

Please let us know your new address and telephone number as soon as possible so we can keep your records up to date.  If you move outside the Practice area, we may not be able to continue to care for you; if you are in any doubt, please check with reception.
